Use this form for any Clear Fork machine at your workplace, community, hotel, or facility. Managers and hosts can use it too. We read every request and add items when they make sense for that location.
We can’t promise every item will be stocked, but real requests from the people using the machine are the best guide we have.
Tip: tell us where the machine is (floor, break room, building) so we know which one you mean.
